INSURANCE
At this time, we are considered an out-of-network provider for all insurances. Cash and credit cards are accepted for payments and are due at the time of psychiatric service. Our staff will gladly provide the paperwork required by your insurer for consideration for reimbursement of fees. Reimbursement rates vary and are strictly between you and your insurance provider.

NEW PATIENT COMPREHENSIVE EVALUATION- $200
NEW PATIENT COMPREHENSIVE EVALUATION FOR ADDICTION TREATMENT (includes eval & 1st follow up)-$300
MEDICATION MANAGEMENT FOLLOW UP- $100
PAPERWORK requests- $25-$75
CANCELLATION POLICY
If you are unable to keep your appointment, please give us a 24-hour notice or you will incur a $25 late cancellation fee. See office policies.
